How do I order the excerpt in writing?

You can get the relevant request form by...

a) ...downloading it from the internet and printing it out         

b) ...requesting it from the Federal Office of Justice (Swiss Criminal Record, Bundesrain 20, 3003 Bern, Switzerland).

c) ...requesting it from familyservice.

Please comply with these instructions for ordering an excerpt from the Criminal Record in writing. You will only be given an excerpt if you follow them:

1. Personal details
   An excerpt from the Criminal Record can only be ordered for oneself (Art. 363 StGB). To avoid mistakes, personal details must be given legibly and in full on the form.
   To save 3 or 4 working days, you can ask to have the excerpt sent directly to familyservice. To do this you must give the address of the relevant familyservice office on the application form under “If delivered to a third party”.
   
2. Charges and payment
   The charge for each excerpt is CHF 20.00 (Ordinance of the FDJP (Federal Department of Justice and Police) on Charges for excerpt from the Criminal Record for Private Individuals).
   
   Charges for requests made from within Switzerland are to be paid to the following account:

   Strafregister
   3003 Bern
   
   Internet payments (telebanking, payment instructions etc.) are not accepted!
   
   The application form must be accompanied by a legible photocopy of an official, current identification document, for example identity card (both sides of new version), residency permit, residence confirmation, foreigner’s permit, driving license, plus a postal receipt confirming that the fee has been paid. These documents will be returned with the excerpt.
   Do not include originals!
   
3. Signature
